Model Prediksi Kebutuhan Transfusi Packed Red Cell Perioperatif pada Operasi Tumor Tulang

Abstract

Latar Belakang: Dua puluh persen dari operasi tumor tulang membutuhkan transfusi darah packed red cell (PRC) intraoperatif, dengan volume transfusi rata-rata 1200 ml. Kekurangan dalam jumlah PRC yang disediakan dapat menimbulkan perburukan kondisi pasien, sedangkan kelebihan permintaan darah dapat menimbulkan kerugian biaya.

Tujuan: Penelitian ini bertujuan membuat model prediksi jumlah kebutuhan transfusi PRC pada operasi tumor tulang berdasarkan faktor-faktor letak, ukuran, karakteristik keganasan tumor, nilai Hb prabedah dan nilai ASA prabedah.

Metode: Penelitian kohort retrospektif ini dilakukan pada pasien dewasa, ASA 1-3 yang menjalani pembedahan tumor tulang tahun 2015- 2018 dan setelah mendapat ijin dari Komite Etik Penelitian Kesehatan FKUI RSUPN Dr. Ciptomangunkusumo. Pasien dengan tindakan core biopsy dan data rekam medis yang tidak lengkap dikeluarkan dari penelitian. Data lokasi, ukuran, dan karakteristik malignansi tumor, konsentrasi Hb preoperatif serta jumlah PRC yang ditransfusikan dicatata dan dianalisis. Analisis bivariat dan multivariat dilakukan dengan menggunakan statistical package for the social sciences (SPSS) versi 21.

Hasil: Analisis dilakukan pada 82 data yang didapat dari rekam medis. Uji bivariat menunjukkan letak tumor, ukuran tumor, karakteristik keganasan tumor, nilai Hb prabedah dan nilai ASA prabedah memiliki hubungan bermakna terhadap kebutuhan transfusi PRC perioperatif. Analisis multivariat regresi linier menunjukan hanya letak tumor dan nilai Hb prabedah yang dapat memprediksi jumlah kebutuhan transfusi PRC perioperatif pada operasi bedah tulang.

Kesimpulan: Letak tumor dan nilai Hb dapat memprediksi memprediksi jumlah kebutuhan transfusi PRC perioperatif pada operasi bedah tulang.
