Penelitian ini menganalisis pelaksanaan program keselamatan pasien ditinjau dari tujuh langkah menuju keselamatan pasien di Puskesmas X Kabupaten Demak. Metode: Penelitian ini menggunakan metode kualitatif dengan pendekatan deskriptif. Subjek penelitian ini adalah informan utama yaitu anggota keselamatan pasien, dan informan triangulasi yaitu kepala puskesmas, kepala tata usaha, ketua UKM, dan pasien. Hasil : Berdasarkan hasil penelitian masih ditemukan adanya insiden, terjadi keterlambatan pelaporan, tidak adanya agenda khusus keselamatan pasien, pelaporan hanya untuk internal, selama pandemi COVID-19 pelaksanaan keselamatan pasien kurang optimal. Simpulan: Tujuh langkah menuju keselamatan pasien di puskesmas X Kabupaten Demak belum terlaksana dengan optimal. This study proves the safety program of the seven steps towards safety at Puskesmas X Demak district. Method: This study uses a qualitative method with a descriptive approach. The subjects of this study were the main informants, namely members of patient safety, and triangulation of informants, namely the head of the puskesmas, the head of administration, the head of the UKM, and the patient. Result : The results of the study, it was still found that there were incidents, there were delays in reporting, there was no special patient safety agenda, reporting was only for internal, during the COVID-19 pandemic the implementation of patient safety was less than optimal. Conclusion : Seven steps towards patient safety at Puskesmas X, Demak district have not been implemented optimally.
