Contact tracing is the main key in breaking the chain of transmission of COVID-19 in an effort to find and detect people who are vulnerable to the COVID-19 virus. The Kedungmundu Health Center is a health center in Tembalang District that is active in carrying out contact tracing for COVID-19. However, the coverage of the low incidence of COVID-19 contacts at the Kedungmundu Health Center is still relatively low. From this, it is necessary to evaluate the context aspect, input aspect, process aspect and product aspect regarding how to implement the COVID-19 contact tracing program. Method: This research is a qualitative research with descriptive approach using in-depth interview method which was selected based on purposive sampling technique. The research subjects as the main informants include the Head of the Puskesmas, the Coordinator of the Contact Tracing Program and the Tracer Team. Meanwhile, Babinsa, Babhinkamtibmas, Semarang City Health Office and the Community became the triangulation of informants. Result: The achievement of contact tracing for COVID-19 at the Kedungmundu Health Center has reached 87%, but for the timeliness of reporting it is still at 67%. The implementation process is still constrained by the lack of community, as well as the number and competence of the tracking team. Conclusion : The COVID-19 contact tracing program at the Kedungmundu Health Center has not been implemented optimally. Many things still require improvement, adjustment and more appropriate planning.
